How to get your medical records

Medical records hold information about you. They are also sometimes called health records.

You'll have separate records for any NHS service you go to including your GP surgery, hospital, dentist or opticians.

Your GP record includes information like any conditions or allergies you have and any medicine you're taking.

To request copies of your medical records please complete the Subject Access Request form and email to OldIrvineRoad_ADOC@aapct.scot.nhs.uk or hand in to the surgery

Please note that while there is no charge for providing a copy of a patients medical record. A repeat request does incur a fee. You will be notified of the fee when you request a further copy.
